Plusieurs femmes vivant avec l\u2019endom\u00e9triose s\u2019y int\u00e9ressent, notamment pour mieux g\u00e9rer des sympt\u00f4mes digestifs qui peuvent accompagner la condition.<\/p>\n<p data-start=\"320\" data-end=\"642\">En effet, certaines manifestations de l\u2019endom\u00e9triose peuvent ressembler \u00e0 celles du <span class=\"hover:entity-accent entity-underline inline cursor-pointer align-baseline\"><span class=\"whitespace-normal\">syndrome de l\u2019intestin irritable<\/span><\/span> : ballonnements, douleurs abdominales, diarrh\u00e9e et\/ou constipation. Cette superposition de sympt\u00f4mes peut rendre la situation difficile \u00e0 d\u00e9coder, et amener \u00e0 explorer diff\u00e9rentes pistes alimentaires.<\/p>\n<p data-start=\"644\" data-end=\"912\">Dans ce contexte, l\u2019approche FODMAP peut \u00eatre pertinente \u00e0 consid\u00e9rer \u2014 particuli\u00e8rement lorsqu\u2019il y a des inconforts digestifs marqu\u00e9s. Cela dit, elle ne s\u2019adresse pas \u00e0 toutes les femmes atteintes d\u2019endom\u00e9triose et m\u00e9rite d\u2019\u00eatre utilis\u00e9e de fa\u00e7on cibl\u00e9e et encadr\u00e9e.<\/p>\n<p data-start=\"914\" data-end=\"1086\">Dans cet article, je te propose de mieux comprendre en quoi consiste l\u2019approche FODMAP, ainsi que certains points importants \u00e0 garder en t\u00eate si tu envisages de l\u2019explorer.<\/p>\n<h3 data-section-id=\"8jxyw8\" data-start=\"1088\" data-end=\"1114\">C\u2019est quoi les FODMAP?<\/h3>\n<p data-start=\"1116\" data-end=\"1556\">Les FODMAP sont des glucides fermentescibles, c\u2019est-\u00e0-dire des sucres qui sont peu absorb\u00e9s dans l\u2019intestin et qui peuvent \u00eatre rapidement ferment\u00e9s par le microbiote intestinal. Elle vise \u00e0 identifier, de fa\u00e7on structur\u00e9e, les aliments qui peuvent d\u00e9clencher ou exacerber des inconforts digestifs.<\/p>\n<p data-start=\"366\" data-end=\"434\">Concr\u00e8tement, cette approche se d\u00e9roule en trois phases distinctes :<\/p>\n<ul data-start=\"436\" data-end=\"542\">\n<li data-section-id=\"1ldniph\" data-start=\"436\" data-end=\"478\"><strong data-start=\"438\" data-end=\"461\">Phase d\u2019\u00e9limination<\/strong> (courte dur\u00e9e)<\/li>\n<li data-section-id=\"1vj23el\" data-start=\"479\" data-end=\"510\"><strong data-start=\"481\" data-end=\"508\">Phase de r\u00e9introduction<\/strong><\/li>\n<li data-section-id=\"2ia23b\" data-start=\"511\" data-end=\"542\"><strong data-start=\"513\" data-end=\"542\">Phase de personnalisation<\/strong><\/li>\n<\/ul>\n<p data-start=\"544\" data-end=\"748\">La phase d\u2019\u00e9limination consiste \u00e0 retirer temporairement les aliments riches en FODMAP, g\u00e9n\u00e9ralement pour une p\u00e9riode limit\u00e9e (environ 4 \u00e0 6 semaines), afin d\u2019observer l\u2019\u00e9volution des sympt\u00f4mes digestifs.<\/p>\n<p data-start=\"750\" data-end=\"1110\">Ensuite vient la phase de r\u00e9introduction, qui est centrale dans la d\u00e9marche. Elle permet de tester progressivement diff\u00e9rents groupes de FODMAP afin d\u2019identifier les tol\u00e9rances individuelles. C\u2019est une \u00e9tape cl\u00e9, qui gagne \u00e0 \u00eatre encadr\u00e9e par une nutritionniste-di\u00e9t\u00e9tiste form\u00e9e \u00e0 cette approche, compte tenu de sa complexit\u00e9 et de la rigueur qu\u2019elle demande.<\/p>\n<p data-start=\"1112\" data-end=\"1282\">Enfin, la phase de personnalisation vise \u00e0 r\u00e9int\u00e9grer le plus d\u2019aliments possible, en fonction des tol\u00e9rances, pour arriver \u00e0 une alimentation vari\u00e9e, adapt\u00e9e et durable.<\/p>\n<p data-start=\"1284\" data-end=\"1488\">Un enjeu fr\u00e9quent, toutefois, est que certaines personnes restent dans la phase d\u2019\u00e9limination pendant de longues p\u00e9riodes \u2014 parfois plusieurs mois, voire ann\u00e9es. Or, ce n\u2019est pas l\u2019objectif de l\u2019approche.<\/p>\n<p data-start=\"1490\" data-end=\"1741\">La phase d\u2019\u00e9limination est, par d\u00e9finition, restrictive et exclut plusieurs aliments qui contribuent \u00e0 la diversit\u00e9 du microbiote intestinal. Prolonger cette phase inutilement peut, \u00e0 long terme, nuire \u00e0 l\u2019\u00e9quilibre digestif plut\u00f4t que de le soutenir.<\/p>\n<p data-start=\"1743\" data-end=\"1936\">L\u2019objectif n\u2019est donc pas d\u2019\u00e9liminer tous les FODMAP de fa\u00e7on permanente, mais bien d\u2019identifier les d\u00e9clencheurs sp\u00e9cifiques \u00e0 <strong data-start=\"1871\" data-end=\"1878\">toi<\/strong>, pour ensuite \u00e9largir l\u2019alimentation autant que possible.<\/p>\n<h3 data-section-id=\"709ziu\" data-start=\"1943\" data-end=\"2036\">Est-ce que toutes les femmes atteintes d\u2019endom\u00e9triose devraient suivre l\u2019approche FODMAP?<\/h3>\n<p data-start=\"2038\" data-end=\"2062\">La r\u00e9ponse courte : non.<\/p>\n<p data-start=\"2064\" data-end=\"2251\">L\u2019endom\u00e9triose se manifeste de fa\u00e7on tr\u00e8s variable d\u2019une femme \u00e0 l\u2019autre. Les sympt\u00f4mes, leur intensit\u00e9, ainsi que la pr\u00e9sence (ou non) d\u2019inconforts digestifs peuvent grandement diff\u00e9rer.<\/p>\n<p data-start=\"2253\" data-end=\"2491\">L\u2019approche FODMAP peut \u00eatre pertinente dans certains contextes \u2014 notamment lorsqu\u2019il y a des sympt\u00f4mes digestifs importants qui s\u2019apparentent \u00e0 ceux du <span class=\"hover:entity-accent entity-underline inline cursor-pointer align-baseline\"><span class=\"whitespace-normal\">syndrome de l\u2019intestin irritable<\/span><\/span> \u2014 mais elle n\u2019est pas indiqu\u00e9e syst\u00e9matiquement.<\/p>\n<p data-start=\"2493\" data-end=\"2575\">Chaque situation m\u00e9rite d\u2019\u00eatre \u00e9valu\u00e9e de fa\u00e7on individualis\u00e9e, en tenant compte :<\/p>\n<ul data-start=\"2576\" data-end=\"2724\">\n<li data-section-id=\"3vtesi\" data-start=\"2576\" data-end=\"2601\">des sympt\u00f4mes digestifs<\/li>\n<li data-section-id=\"7e55to\" data-start=\"2602\" data-end=\"2661\">du contexte global (hormonal, inflammatoire, alimentaire)<\/li>\n<li data-section-id=\"1d55l3k\" data-start=\"2662\" data-end=\"2684\">des habitudes de vie<\/li>\n<li data-section-id=\"10xyvks\" data-start=\"2685\" data-end=\"2724\">et de la relation avec l\u2019alimentation<\/li>\n<\/ul>\n<p data-start=\"109\" data-end=\"369\">Avant d\u2019envisager une approche restrictive comme la FODMAP, il est souvent pertinent d\u2019explorer d\u2019autres strat\u00e9gies alimentaires plus globales et moins contraignantes. Dans plusieurs cas, ces ajustements peuvent d\u00e9j\u00e0 contribuer \u00e0 am\u00e9liorer le confort digestif.<\/p>\n<p data-start=\"371\" data-end=\"655\">Par ailleurs, certaines femmes observent que leurs sympt\u00f4mes digestifs fluctuent au fil du cycle menstruel. Lorsqu\u2019elle est pertinente, elle gagne \u00e0 \u00eatre utilis\u00e9e de fa\u00e7on encadr\u00e9e, avec une nutritionniste-di\u00e9t\u00e9tiste form\u00e9e \u00e0 cette approche.<\/p>\n<p data-start=\"355\" data-end=\"634\">Il est aussi essentiel de rappeler que la phase d\u2019\u00e9limination est temporaire.
